You will find links to all of the Online Camper Forms: the Camper Information Form, the Health Form, and the Transportation Form. All forms MUST be filled out for each child attending by May 15th. |

Must be completed once for each individual child attending.
At the completion of the Online Health History Form, you will be asked to print out a 5-page packet (link also available below) that will need to be completed and returned by mail to the Camp Office. This packet includes: the Medical Exam and Immunization form to be completed by your child's primary care physician (if you have not already scheduled your child's camp physical, you should do so!), and a form that asks for a recent photo of your child, and copies of your health insurance cards.
